Diocese policies and guidelines for volunteers
From the Risk Management Department of the San Jose Diocese: All volunteers must complete and sign a Volunteer Activity Waiver Form; One form per person, per ministry and on an annual basis. No matter what age, their duties, or duration etc. This form is in case of any accidents- not stating that you can not make a claim because you always can. English form is to be used for all languages until the Diocese prepares other language forms.
Download the SJMV Volunteer Activity Waiver Form in English. This will cover any volunteer at both the church and school for up to a year. If you serve more than one ministry, print out the form and complete the form leaving name of ministry and ministry contact name open. Make copies of your complete form and then fill in the name of ministry and ministry contact name. Each ministry coordinator must keep a hard copy of this form and send a copy to the Diocesan Risk Management Department as stated on the form. All forms will be used for liability purposes only. The annual deadline for Ministries Coordinators is September 30th. Sending all copies to the Diocese (see form) by October 1st annually.
